Recorded Outcomes Form

Some youth services have adopted a standardised recording form for recorded (and in some cases accredited) outcomes. This form would not be used routinely, but only when a young person has achieved something significant. Information from this form would be kept at the youth centre but would inform the regular data collection from head office.
