Using Sessional Recording Sheets

One of the most frequent concerns from workers is that recording outcomes involves a lot of form filling, and extra work. Using the service’s existing sessional evaluation sheets as the basis for recording outcomes reduces this. However sessional recording sheets on their own do not provide enough evidence of the young person’s achievements, and so they need to be supplemented by either evidence to be found in the young person’s folder, or a recorded outcomes sheet.
